Intestinal Intussusception Induced by Lymphosarcoma



Abstract
This is a case report of ileo-ileo-cecal intussusception due to lymphosarcoma of the terminal ileum.
Patient is 2 2 year old girl who had sudden colicky abdominal pain with tender mass over right lower quadrant of abdomen since 5 hours prior to admission to the hospital. But she did not pass tarry stool. There noted sausage-like mass with severe tenderness over right lower quadrant of abdomen. Clinical diagnosis of intussu:c.-ption was made on the basis of physical and X-ray findings.
Exploratory laparotomy showed ileo-ileocecal intussusception with the findings of coin-like subserosal hard mass on the anti-mesenteric border of terminal ileum which was measured approximately 15 cin proximally from ileocecal valve. Further exploration revealed several scattered mesenteric lymph-node enlargement which showed reactive hyperplasia on frozen section. The mass was diagnosed as sarcoma grossly. No signs of gross metastasis to other viscera or regional lymph-nodes were demonstrated. Segmental ileal resection and end-to-end anastomosis was made. Because of rarity of this pathology in children, its surgical and radiological management were reviewed through the literature. The most dreadful complication in the later course of this disease seemed to be leukemic transformation in its course. This is more so in young children like our case.
